Health Portal & Healow App

The Health Portal is a secure tool that lets you communicate with your doctor and care team, securely and safely view health records and information, any time, 24 hours a day from a computer or smart phone/tablet with the Healow App. You will be able to update personal data, access your appointments, lab results and vitals, request medications refills and participate in virtual visits with TeleVist.

How Do I Sign Up?

Our center staff offer patients access to the Health Portal upon patient registration. Patients will receive a welcome email after they provide us their email. The email will have the subject line:

Portal login information from your doctor's office

It will include:

  • Health Portal URL: https://health.healow.com/PatientHealthPortal
  • Directions on setting up your account 
  • After setting up your account, visit the Health Portal to download the free Healow App for mobile use
  • Find us using our unique practice code on the Healow app: FAFJAD

If you have questions or need access, please call our center and a member of our team will assist you in getting a username and password.

I Have An Email Address

That’s great, please share with your medical center so they can place it in your patient records.

I Don’t Have An Email Address

An email address is required to have electronic access to your medical information via The Health Portal.

Televisit Instructions

See your doctor with a Televisit

Partners in Primary Care is making it so you can continue getting the care you need from home. Whether you choose to use your computer, tablet or mobile phone, just follow the step by step instructions below to get started.

Scheduling your Televisit from your computer or tablet

Health Portal Appointment Email Confirmation
  • You will receive an email confirmation soon after the appointment is scheduled. An email and text will also be sent to you 30 minutes before your appointment as a reminder along with a link to start your TeleVisit. The email will come FROM: no-reply@eclinicalmail.com with a SUBJECT: Telemed Appointment Confirmation from your Doctor’s Office. See email confirmation image.
  • You must have access to the Health Portal to schedule a virtual visit. If you already have access, proceed to Step 2. If you do not have access, please call our center, and a staff member will help you register.
    Important note: You will need to provide an email address to register. If you do not have an email address or internet access, contact us to set up a phone visit.
  • Go to www.PartnersInPrimaryCare.com, click Health Portal at the top of the webpage and then login.
  • Click on Appointments in the left-hand menu then choose TeleVisit in the Appointment Type drop down menu. See the image below.

Beginning your Televisit from your computer or tablet

  • A few minutes before the scheduled time of your visit, log in to the Health Portal. If you can’t access the Portal, you can also begin your appointment through the reminder link sent to your email.
  • Click “Join TeleVisit” in your patient dashboard (or email reminder).
  • Fill out the patient questionnaire and provide requested personal information.
  • Click “Proceed” to check your computer’s speed and ability to support a TeleVisit. If your computer does not have a webcam (most do), you will need to attach one.
  • Click “Start TeleVisit” to enter the virtual waiting room.
  • Your physician will begin the face-to-face TeleVisit, and disconnect when the visit ends.

Televisit from your computer or tablet

Scheduling your Televisit from your mobile phone or tablet

  • You must have access to the Patient Portal via the Healow mobile app to schedule a virtual visit from your phone. If you already have access, proceed to Step 2. If you do not have access, please call our center, and a staff member will help you register for the Patient Portal and download the Healow app. The free Healow app is available to download via Apple’s App Store or Google Play which you can access through the Patient Portal.

    Important note:

    • You will need to provide an email address to register. If you do not have an email address or internet access, contact us to set up a phone visit.
    • Upon first use, you only need to enter our practice code for the Healow app: FAFJAD in the area outlined in navy blue in the app screen image shown below.

  • Enter your login information into the app. If you have any issues with logging in, please call our office for assistance.
  • Submit your Televisit appointment request by composing and submitting a message to your center within the Healow app. From the main screen, click the blue “+” button, then click the green “compose” button with the pen and paper icon. You will then select the message type, enter the message details and click send on the top right corner of the screen.
  • You will receive an email confirmation soon after the appointment is scheduled. An email and text will be sent to you 30 min before the appointment as a reminder along with the link to start your TeleVisit. The email will come FROM: no-reply@eclinicalmail.com with a SUBJECT: Telemed Appointment Confirmation from your Doctor’s Office.

* Text notifications require a cell phone number on file.

Beginning your Televisit from your mobile phone or tablet

  • You have three options to start your TeleVisit a few minutes before the scheduled time of your visit. Important note: Choosing the link sent via email or text does not require you to login or to download an app. Just simply click on the link.

  • Click “Start TeleVisit” to enter the virtual waiting room.
  • Your physician will begin the face-to-face TeleVisit, and disconnect when the visit ends.

Frequently Asked Questions

  • Am I eligible to enroll?

    All patients are eligible to enroll with an email address and to sign the General Consent form.  You will receive a welcome email and a print out of your sign-on credentials (user name and temporary password) before leaving the office. 

  • How do I get a User Name and Password?

    If you are in our center registering for the health portal you will be provided a print out of your user name and temporary password by the center staff after web enabling you in our system.

    If you are trying to access the Health Portal from home, you will need to call our Health Portal Hotline at 864-465-6259 and answer several questions to verify your identity. Once you have answered the patient validation questions you will receive an email with information about your user name and password.

  • Can I change my User Name?

    Yes, a member of our center staff can change your User Name for you.

  • What happens if I forget my user name?

    If you forgot your user name and you have signed in at least once, you can retrieve your user name through The Health Portal.

    1. Click The Health Portal link below
      https://health.healow.com/PatientHealthPortal
    2. Once you have landed on The Health Portal home page, click on ‘Trouble logging in’ under the password field:

    1. Select ‘Forgot User Name’ and click ‘Next’.
    2. In the ‘User Name Recovery Help’ screen, enter your email address and click ‘Next’. You will receive a confirmation message indicating that you will receive an email to assist you with resetting your user name.
    3. Return to The Health Portal home page and follow the emailed instructions to reset your user name.
  • How do I get a password?

    You will receive a temporary password at the time of registration.  The temporary password will need to be replaced when you activate your account.

  • What happens if I forget my password?

    If you forgot your user name and you have signed in at least once, you can retrieve your user name through The Health Portal.

    1. Click The Health Portal link below

    2. Once you have landed on The Health Portal home page, click on ‘Trouble logging in’ under the password field:

  • Why do I have to accept two consents on the website?

    One of the consents you must agree to is our vendor consent.  My Health Information (patient portal) is powered by a vendor called eClinicalWeb, LLC (“ECW”).  This consent outlines the Terms of Use Agreement (“Terms of Use” or “Agreement”) defines the terms and conditions under which ECW makes the Site available. The second is a medical center practice consent: addressing issues and policies of the practice in exchanging information with patients electronically. It is important to have your permission to communicate with you via The Health Portal, it’s another way to protect your patient information.

  • Can I change my address or phone number on the health portal?

    Yes, you can submit changes through the portal under My Account > Personal Information.

  • What if I have questions about my lab results?

    Lab results will be available on the Health Portal.  However, we recommend that you speak to your primary care physician and medical team to answer all your questions.
